Functional Assessment Tool

Claim Readiness Report

This tool helps you document how your condition affects your daily life — in language that VAC and CRA actually use. It covers Activities of Daily Living (VAC Table of Disabilities), mental functions (CRA DTC), and work capacity.

When you finish, you'll receive:

  • A plain-language narrative using VAC and CRA terminology (for your doctor, navigator, or representative).
  • A structured summary showing which CRA DTC categories may be flagged (marked restriction, significant limitation, or cumulative effect).
  • A downloadable text report and structured data file you can keep or share.

What you'll be asked about

Module 1 — Daily Living

4 pages · Walking, dressing, feeding, hygiene, arms, hearing, speaking, vision, household tasks, memory

Module 2 — Mental Health

3 pages · Diagnosis, symptom severity, mental functions necessary for everyday life

Module 3 — Physical Function

2 pages · Mobility, upper limb, pain levels, flare-ups

Module 4 — Work & Earnings

1 page · Work status, limitations, hours, absences, income impact

Nothing is sent to VAC, CRA, CPP, or any organization. You decide what to download, print, or share.

Do not enter sensitive documents, SIN numbers, medical files, military files, or financial records.

Takes approximately 15–25 minutes. You can stop at any point and restart.

This tool provides guidance and navigation support only. It is designed to help veterans describe their symptoms, functional limitations, and daily impacts using language commonly recognized by Veterans Affairs Canada (VAC), the Canada Revenue Agency (CRA), and related benefit programs.

This tool does not assess eligibility, guarantee approval, influence benefit outcomes, or replace professional advice. It does not provide legal, medical, clinical, financial, emergency, or crisis services.

All benefit decisions, reassessments, reviews, and appeals are made solely by the responsible department, tribunal, insurer, or benefit provider. Veterans should consult qualified medical and professional practitioners for diagnosis, treatment, and completion of official forms.

If you are in immediate danger or thinking of harming yourself, please call 911. For crisis support anywhere in Canada, call or text 988. Veterans, former RCMP members, families, and caregivers may also contact the VAC Assistance Service at 1-800-268-7708. VANCA is not an emergency service.
